Family Support Team
Here to provide support, advice and peace of mind to families and carers of patients.
Peace of mind
We know that as well as affecting your health, a life threatening illness can also present a range of new challenges; often financially, socially and emotionally.
The Family Support Team is available Monday to Friday from 9am to 5pm both in the Hospice and for home visits.
The team includes:
- Social Workers
- Welfare Rights Advisors
- Children’s Support Worker (Unicorn)
- Complementary Therapists
What can the Team Help With?
The team can help with a range of issues or problems. Some of the most common issues they deal with are:
- Benefits advice
- Help with completing forms
- Carers support
- Liaising with other agencies on your behalf
- Emotional and psychological support
How do I Contact the Family Support Team?
You can ask your specialist nurse to refer you to the team, who will then contact you directly to arrange to visit you. You can also see members of the team at one of our Day Hospices or at one of our resource centres or by ringing 01633 851051 or emailing familysupportteam@stdavidshospicecare.org
