Have peace of mind that the St David’s Hospice Care Community Team can look after and support you in the comfort of your own home.
Bringing care home
Many patients would prefer to stay at home during their illness. The St David’s Hospice Care Community Team can offer you specialist advice and support in your own home.
St David’s Hospice Care employs a large team of other professionals who can be involved in your care if required: social workers, complementary therapists, occupational therapists and our spiritual care team. All these professionals are here to support you and your family. They can also refer you to our Day Hospice or arrange admission to our In-Patient Unit, if necessary.
Our community team works closely with other professionals who may be involved in your care including GPs, District Nurses and hospital staff. Your GP, District Nurse or hospital team can refer you to our service.
Our Clinical Nurse Specialists:
- Can discuss issues that may be having an impact on your life, whether they are physical, emotional, spiritual or social.
- Have a good knowledge of what may help, including medication which may help to ease symptoms such as pain, nausea or breathlessness.
- Have time to discuss your thoughts and feelings and give information to support you to make choices such as planning for the future.

Hospice at Home
Your Clinical Nurse Specialist may feel it would be helpful to you and your family if a referral is made to our Hospice at Home service. This service can offer a nurse or Health Care Support worker to care for you overnight or during the day.
The service is responsive to patients' individual requirements, and our aim is to help patients remain in their own home if that is their choice.
If you are referred to this service, further information and a booklet will be provided at the time of referral.
Frequently Asked Questions
How long will I have the Hospice at Home care?
You and your needs will be regularly reassessed by your Clinical Nurse Specialist. Normally care is provided for 2-4 weeks and then your hours of care will be reviewed.
What type of care at home can you provide?
Our Hospice at Home Nurses can provide:
- General Care
- Pressure Area Care
- Oral Care
- Continence/Toileting
- Moving and Handling
- Syringe Drivers
- Symptom Management
- Psychological Support for Patients and Families
Will I always have the night cover that I have asked for?
Depending on the level of care you need, sometimes we may need to offer an alternative night.
Do you offer 7 nights a week cover?
Yes we do this when people are reaching the end of their lives, this is normally for 2-4 weeks.
Will it always be the same nurse?
We will do our best to try to allocate the same nurse to you. However, this may not always be possible.
