Create A Tribute
There are a few designs to choose from that allow you to personalise your tribute and make it unique to the person you'd like to remember.
Once ordered, your slate tribute will be lovingly created and placed under the chestnut tree in our hospice memory garden, where it will stay for one year. This gives you the opportunity to come and visit the tribute in your own.
Importantly, the purchase of every tribute helps to support the work of St David's Hospice Care, so that your contribution is helping current and future patients and their families to get the care and support they need, which is a truly lovely gift.
How It Works
One the next page you will need to decide on the design and wording you would like for your slate.
First choose one of the below shapes, either diamond or circle:
Then choose the icons you would like added to the left and the right of the slate:
Either Dove, Heart, Butterfly or Rose
All plaques will begin with 'In loving memory of' followed by your personalised text.
You will need to provide:
- Name of your loved one to be remembered.
- Dates (XXXX) - (XXXX) e.g. 1961 - 2025.
- Short tribute message.
- From.
We suggest no more than 25 characters for the 'tribute message' and 'from' fields to avoid overcrowding.

Visiting Your Tribute
Please allow at least 2 weeks from the date of your order before visiting. This allows us enough time to lovingly craft your custom plaque and place it in the memory garden.
All tributes will be located in a reserved area in the shade of the chestnut tree and is accessible 24/7.
The memory garden is located behind our In Patient Hospice and is accessed via a compacted earth ramp from the rear car park.
Why not visit during our cafe opening hours and treat yourself to a coffee and cake during your visit. The cafe is located inside the In Patient Hospice and is open 10am to 4pm Monday to Friday and 11am to 3pm on weekends and bank holidays.

Afterwards
Your tribute will stay in the memory garden for 1 year. After this your plaque will be stored at our Day Hospice for collection. You are more than welcome to collect your plaque before 1 year if you'd like to take it home.
